Despite the difficult financial situation in the country, dictating austerity conditions in all sectors without exception, the Russian government is seeking funds to support healthcare facilities. So, starting from the second half of the year current year, the medical organizations of the Samara region have a unique opportunity to continue the modernization program that was implemented from 2011 to 2013. In this regard, a separate source of financing is formed in the Territorial Fund of Compulsory Health Insurance as part of the normalized insurance stock (NHA).

The money in question is accumulated from the application of sanctions for poor quality medical care. In other words, these are funds returned to the Fund's budget from those who have fined medical institutions... They are redistributed according to certain criteria.

Regulatory documents imply three main directions for spending NHA funds. They can be spent on:

1) advanced training of medical personnel;

2) maintaining the operability of previously purchased medical equipment in a healthcare facility;

3) purchase of new medical equipment.

Treat by standards

As a reminder, in 2016, in connection with emergencies at two facilities of the regional healthcare industry - the Samara Clinical Geriatric Hospital and the Chelno-Verkhinsk Central Regional Hospital - TFOMS has already made an urgent decision on an application for the allocation of funds to eliminate the consequences of accidents ...

In the Chelnovershinsky district, a medical facility suffered as a result of a natural disaster. When assessing the damage, the corrugated roof of the transition from the infectious diseases department to the main building (area 212 square meters, length 30 meters) 465 thousand rubles were allocated from the funds of the Territorial Fund of the CHI. Repairs have been completed, the hospital is operating normally.

The Samara Clinical Geriatric Hospital has been allocated 3 million 700 thousand rubles for repair work. There, as a result of a breakthrough in the cold and hot water supply system, the premises of the 1st floor were flooded, where, among other things, the emergency department and intensive care wards are located.

However, the main direction of expenses of the normalized safety stock, according to the deputy director of the TFOMS of the Samara region Vladislava Romanova, is the equipping of medical institutions with the necessary medical equipment to provide medical care to the population under the compulsory medical insurance program. It is important that the quality of rendering medical services must comply with the current standards in all healthcare facilities of the region without exception.

System transparency is the basis for decision making

As Vladislav Romanov explained, there are two groups of criteria that determine the inclusion of medical organizations in the plan of measures for the additional education of medical workers, repairs and the purchase of medical and diagnostic equipment at the expense of the NHS.

- General criteria imply the work of health facilities in the territorial CHI program, confirmed by all necessary documents, and the timely provision of a correctly executed application, -tells Vladislav Romanov ... - When considering an application, it is also taken into account that the organization has income from the implementation of the territorial compulsory medical insurance program, the staffing of the medical institution with doctors and paramedical personnel and the proportion of doctors with qualification categories. Additional criteria include the specifics of a medical organization (provision of planned, emergency, specialized and / or high-tech medical care, the status of a regional vascular center, assistance to the rural population, etc.). For all of the above, according to the approved grid, points are awarded to the medical institution, which form the basis for making a decision.

In total, about 78 million rubles are provided for the implementation of these directions in the NSC in 2016. Almost 64 of them - for the purchase of equipment, 13 - for the repair of equipment. Meetings on the inclusion of medical institutions in the Plan are held in the regional Ministry of Health with the participation of TFOMS (three meetings have already taken place). Note that next year the Program will continue.

So, among the measures already implemented - the repair of a mobile digital X-ray machine in the Syzran Central City Hospital in the amount of about 1.5 million rubles and a computer tomograph in the Samara city hospital named after Pirogov for the amount of about 2 million rubles. The equipment was also repaired in the Neftegorsk Central Regional Hospital, Togliatti GKB No. 2 named after Banykin, Krasnoyarsk Central District Hospital, Bezenchukskaya Central District Hospital and Novokuibyshevsk Central City Hospital.

With regard to the purchase of the necessary medical equipment, in the Samara City Children's Clinical Hospital No. Ivanova purchased an electrosurgical high-frequency apparatus with a vessel ligation function worth about half a million rubles, and a fibrogastroscope for neonatal and pediatric examinations worth about 1.5 million rubles and a light source for it worth almost 150 thousand rubles for Togliatti City Children's Hospital No. 1. 1. The procedure for using the funds of the normalized insurance stock of the territorial compulsory medical insurance fund (hereinafter referred to as the Procedure) has been developed in accordance with Federal Law dated November 29, 2010 N 326-FZ "On Compulsory Medical Insurance in the Russian Federation" (hereinafter referred to as the Law) and determines the rules for use by the territorial fund of compulsory medical insurance (hereinafter referred to as the territorial fund) the means of the normalized insurance stock of the territorial fund.

2. The normalized insurance stock of the territorial fund (hereinafter referred to as the normalized insurance stock) is formed as part of the budget of the territorial fund to ensure the financial stability of compulsory medical insurance.

3. The size and purposes of using the funds of the normalized insurance stock are established by the law on the budget of the territorial fund. The amount of the normalized insurance stock should not exceed the average monthly amount of the planned income of the territorial fund for the next year for the financial support of the territorial compulsory medical insurance program. The purposes of using the funds of the normalized safety stock are established in accordance with paragraph 4 of this Procedure. The funds of the normalized insurance stock are accounted for on the account for recording the funds of compulsory health insurance. The amount of the normalized insurance stock is included in the total amount of financial support for the territorial compulsory health insurance program.

4. The funds of the normalized safety stock are used for:

1) financial support for the implementation of territorial compulsory health insurance programs in the form of additional financing of medical insurance organizations;

2) payments for medical care provided to insured persons outside the territory of the constituent entity of the Russian Federation in which the policy of compulsory medical insurance was issued, in part:

a) reimbursement to other territorial funds of the costs of paying for the cost of medical care provided to insured persons outside the territory of the constituent entity of the Russian Federation in which the policy of compulsory medical insurance was issued, in the amount provided for by the basic program of compulsory medical insurance;

b) payment of the cost of medical care provided medical organizations of a constituent entity of the Russian Federation to persons insured on the territory of other constituent entities of the Russian Federation, with the subsequent restoration of funds into the standardized insurance stock as costs are reimbursed by other territorial funds.

5. As part of the standardized insurance stock, funds may be provided for incentive payments to medical organizations for meeting the target values \u200b\u200bof the availability and quality of medical care established by the territorial fund. At the same time, the funds of the normalized safety stock allocated for these payments may not exceed 10 percent of the total amount of the normalized safety stock.

6. As part of the standardized insurance stock, funds may be provided for the payment of remuneration to medical insurance organizations for fulfilling the conditions stipulated by the agreement on financial support compulsory health insurance. The list of indicators for calculating remuneration for medical insurance organizations is determined by the territorial compulsory medical insurance fund. At the same time, the funds of the normalized safety stock allocated for these payments may not exceed 5 percent of the total amount of the normalized safety stock.

7. In case of exceeding the amount of funds established in accordance with the Law for an insurance medical organization for paying for medical care due to increased morbidity, an increase in tariffs for paying for medical care, the number of insured persons and (or) a change in their structure by sex and age, the territorial fund makes a decision on the provision or refusal to provide an insurance medical organization with funds missing from the standardized insurance stock to pay for medical care. The funds provided from the standardized insurance stock are directed by the medical insurance organization exclusively to pay for medical care to insured persons under the territorial compulsory medical insurance program.

8. According to part 8 of Article 34 of the Law, the procedure for making payments for medical care provided to insured persons outside the territory of the constituent entity of the Russian Federation, in which the policy of compulsory medical insurance was issued, at the expense of the normalized insurance stock is determined by the rules of compulsory medical insurance.

9. Control over the targeted use of the normalized insurance stock sent to medical insurance organizations is carried out in accordance with the legislation of the Russian Federation.

From 2019, the compulsory medical insurance fund will include funds in the standardized insurance stock of Terfondov to combat the personnel shortage in the primary care. Now he is going to finance medical care provided outside his native region, additional education for doctors, as well as the purchase and repair of medical equipment.

The Ministry of Health proposes to form the standardized insurance stock (NHA) of territorial compulsory health insurance funds (CHI) in a different way. Now it is intended "for the implementation of territorial compulsory medical insurance programs, payments for medical care provided to insured persons outside the territory of the subject of the Russian Federation, in which compulsory medical insurance policy, the organization of additional professional education for medical workers on training programs, the purchase and repair of medical equipment. " In 2019–2024, it will also include funds for eliminating the personnel shortage in clinics providing primary medical care. The corresponding draft law amending Article 51 of the Federal Law "On Compulsory Health Insurance in the Russian Federation" has been published on the website of the Russian government.

Where to get the funds and how they will be used by medical organizations must be approved by the Ministry of Health in an additional order. , now the normalized insurance stock of TFOMS is formed from the budget revenues of the territorial fund (FFOMS subventions), money intended to pay for medical care to residents of other regions, as well as part of the funds from penalties imposed on clinics based on the results of inspections.

The government also ordered to submit to the State Duma a draft budget for the Federal MHI Fund for next year and for the planning period 2020 and 2021. According to it, the formation of the standardized safety stock of the FFOMS is planned annually in the amount of 3.1 billion rubles.

For the next year, the revenues of the MHIF are provided in the amount of 2,098.2 billion rubles, for 2020 - 2,349.9 billion rubles, for 2021 - 2,495.8 billion rubles (the budget of 2018 - 1,887.9 billion rubles). It is assumed that expenses for 2019 will amount to 2,190.4 billion rubles, in 2020 - 2,350.5 billion rubles and in 2021 - 2,501.5 billion rubles (2018 budget - 1,994.1 billion rubles) ...

Instructions to the head physician: how to get the funds of the normalized safety stock

APPROVED by the Board of the Federal CHI Fund (Minutes dated 10.21.99 N 8)

1. The Regulation on the use of temporarily free financial resources and the standardized reserve stock of territorial CHI funds (hereinafter referred to as the Regulation) has been developed in order to streamline the activities of territorial CHI funds on the use of temporarily free financial resources and standardized insurance stock for placement in bank deposits and investing in state securities (hereinafter - GS), exclusion of cases of unjustified diversion of compulsory health insurance funds, strengthening control over their rational use.

2. The Regulation has been developed taking into account the recommendations of the Accounts Chamber of the Russian Federation based on the results of the audit of the activities of the Federal and territorial CHI funds, in accordance with the Civil Code of the Russian Federation, the Federal Law “On Banks and Banking Activities” dated 02.12.90 N 395-1 (as amended by Federal laws from 03.02.96 N 17-FZ, from 31.07.98 N 151-FZ), the Law of the Russian Federation "On health insurance of citizens in the Russian Federation" dated 28.06.

91 N 1499-1 (as amended by the Law of the Russian Federation of 02.04.93 N 4741-1, the Law of the Russian Federation "On tax on profit of enterprises and organizations" of 27.12.91 N 2116-1, the Decree of the President of the Russian Federation of 24.12.93 N 2288), by the Decree of the President of the Russian Federation of 06.02.98 N 136 "On measures to stabilize the financing of the compulsory health insurance system", the Temporary Procedure for Financial Cooperation and Spending in the System of Compulsory Health Insurance of Citizens, approved by the Federal MHI Fund on 19.08.

3. The territorial CHI fund, as an independent state non-profit financial and credit institution, has the right to carry out financial and credit activities to ensure the financial stability of the compulsory health insurance system, including those related to the placement of temporarily free financial resources and standardized safety stock in bank deposits, government securities.

4. Temporarily free financial resources of the territorial fund are earmarked funds remaining at the disposal of the territorial CHI fund after the implementation of: - financing of compulsory medical insurance in full; - subsidizing branches with insufficient financial resources until the per capita standard established in the territory is reached; - formation of a standardized safety stock in the prescribed manner; - implementation of management functions; - financing of other activities not prohibited by the regulatory legal acts of the Russian Federation on compulsory health insurance.

5. Placement of temporarily free financial resources and standardized insurance stock in bank deposits, government securities is carried out by the territorial fund in order to protect them from inflation and to ensure stable financing of compulsory health insurance.

6. The conditions for placing temporarily free financial resources and standardized insurance stock in bank deposits, government securities by the territorial CHI fund are: - no debt on financing the territorial compulsory health insurance program; - placement of funds on a short-term basis.
