Form SZV-4-1 “Individual information on the length of service and accrued insurance contributions for compulsory pension insurance of the insured person” was generated in the following situations:

  • if the insured person in the billing period (in the reporting year) had any conditions for the appointment of an early retirement pension, which are reflected in the table “”, or if it is necessary to allocate separate periods of child care, etc.;
  • if the insured person had several periods of work at this enterprise in the billing period (reporting year), i.e. the employee was hired and fired several times.

If the employee does not have the above conditions, then these employees were grouped into a single form - form SZV-4-2 (list form).

Rules for filling out form SZV-4-1

When submitting information, starting from 2002, the policyholder (employer) filled out form SZV-4-1 when submitting information about the insured person who, during the billing period, had any conditions for the appointment of an early retirement pension, reflected in the table “Work experience for reporting period”, or if necessary, highlighting separate periods of work, periods of child care, etc.;

If, when filling out the form, information about the insured person’s length of service does not fit into the number of lines established by the form, filling out the form continues on the reverse side of the sheet. Wherein:

New Form of SZI-ILS

Order of the Ministry of Labor of the Russian Federation dated January 09, 2019 N 2n approved a new form “Information on the status of the individual personal account of the insured person” (form SZI-ILS).

The new form was approved due to the fact that from January 1, 2019, the procedure for providing information to insured persons by the Pension Fund of the Russian Federation has been clarified. In addition, the Ministry of Labor of Russia has been designated as the body authorized to approve the form for providing information.

Order of the Ministry of Labor of Russia dated January 9, 2019 N 2n “On approval of the form of information on the status of the individual personal account of the insured person” comes into force on March 17, 2019.

Form SZV-Experience

The SZV-STAZh form was approved by Resolution of the Pension Fund of the Russian Federation dated January 11, 2017 No. 3p.

The SZV-STAZH form is created by policyholders for persons who are in an employment relationship with the policyholder (including those with whom employment contracts have been concluded) or who have entered into civil law contracts with him.

Section 3 of the SZV-STAZH report shows:

  • Last name, first name and patronymic of each insured person (columns 2,3 and 4);
  • work periods (columns 6 and 7);
  • SNILS (column 5);
  • codes of territorial and special working conditions (columns 8 and 9);
  • calculation of insurance experience (columns 10 and 11);

When is the SZV-STAZH form due?

The “Initial” information type means that the report for 2017 is being submitted for the first time.

The “Corrective” information type is required to be submitted when there were errors in the report with the “Initial” type that did not allow the data to be posted to individual personal accounts of insured persons (for example, if there were errors in SNILS).

We believe it is advisable to pay special attention to the type of information “Pension assignment”. The SZV-STAZH report with this type of information will need to be submitted to the Pension Fund of the Russian Federation for a person entering an insurance pension within three calendar days from the date the insured person contacts the policyholder.

Form EDV-1

Form EDV-1 “Information on the policyholder transferred to the Pension Fund for maintaining individual (personalized) records” is accompanying. with a form.

Information in the form SZV-STAZH, SZV-ISKH, SZV-KORR is formed into packages of documents. One package contains one file and is submitted at the same time as the EDV-1 form. Document packages may include documents of only one name and one type.

EDV-1 is a document containing information in general about the policyholder.

For example, in this form you need to summarize information about the total amount of accrued and paid insurance premiums for the reporting period.

EFA-1 includes the following sections:

Details of the policyholder submitting the documents;

Reporting period;

List of incoming documents;

Data in general for the policyholder;

The EFA-1 form may contain the following types of information:

original;

corrective - submitted if you need to correct the data in section 5 of the EFA-1 form;

canceling - submitted if it was necessary to cancel the data in section 5 of the EFA-1 form.

There is no deadline for submitting the EFA-1 form, since it is an accompanying incoming document. In other words, if, for example, the SZV-STAZH report is submitted, then it must be sent to the fund along with the EDV-1 form.

Form SZV - KORR

The SZV-KORR form will need to be filled out and submitted to the Pension Fund of the Russian Federation to adjust the data recorded on the individual personal accounts of insured persons based on previously submitted reports.

The SZV-KORR form is submitted by the policyholder to make changes to the previously submitted data. It is submitted at the initiative of the policyholder at any time and there is no deadline for delivery here either.

Types of information:

CORR - if the information on the individual personal account of the insured person is adjusted (for example, data on earnings (remuneration), income, amount of payments and remunerations of the insured person is replaced, or data on accrued and paid insurance premiums is supplemented).

OTMN - on the basis of the cancellation form, the data recorded on the ILS (individual personal account of the insured person) on the basis of reporting, the data of which is being adjusted, will be cancelled;

SPECIAL - a special type of information with the help of which it will be possible to submit data on the insured person, information about whom was completely absent in the previously submitted reports.

The SZV-KORR form can be submitted to the territorial division of the Pension Fund for adjustment of any period before 2017.

Form SZV-ISH

The SZV-ISH form is filled out only for reporting periods up to and including 2016, if reporting deadlines were violated during these periods.

These are periods when physicists did not submit reports at all, but now it has been decided to report for past periods.

In the SZV-ISH form for past periods you will need to indicate:

  • information about the policyholder;
  • information about the insured person;
  • reporting period;
  • information on the amount of payments and other remuneration accrued in favor of the insured person;
  • information on accrued insurance premiums for the reporting period.

There is no deadline for submitting the temporary storage warehouse-iskh form. The policyholder may submit this form at any time he deems necessary.

Form SZV-M PFR

Organizations and individual entrepreneurs are obliged monthly and no later than the 15th day of the month, submit to the Pension Fund of the Russian Federation the following information about each employee using the SZM-M form:

1) insurance number of an individual personal account - SNILS;

2) last name, first name and patronymic;

3) identification number (if available) - TIN.

When reporting the SZV-M form indicates the reporting period - the previous month.

Form SZV-M “Information about insured persons” presented by employers in relation to insured persons working in an organization under an employment contract or a civil law contract and for which insurance premiums are calculated.

In the absence of contracts concluded with employees, including with the head of the organization, who is the only participant (founder), member of the organization, owner of their property, the SZV-M form is not submitted.

Failure to submit on time or submission of incomplete or unreliable information is subject to a fine of 500 rubles in relation toeveryonethe insured person.

Information about the sole founder

NYou don’t need to take SZV-M to the sole founder, if:

The founder does not receive a salary;

No employment or civil law contract was concluded with him.

If persons work under an employment contract, reporting is presented regardless of the actual payment and other remuneration for the reporting period in favor of the above persons.

In relation to persons working under civil contracts, reporting is submitted subject to the accrual of insurance premiums for remunerations paid under such contracts.

Selfless fulfillment works or provision of services, provision of other support is possible within the framework of charitable activities. Payments here do not apply to insurance premiums.

At the same time, it was indicated that the submitted form SZV-STAZH is accompanied by the inventory EDV-1 “Information on the policyholder transferred to the Pension Fund of the Russian Federation for maintaining individual (personalized) records.” We will tell you how to create such an inventory in this material.

Filling out EDV-1

When preparing the SZV-STAZH form in electronic form, the EDV-1 inventory is generated automatically and included in the set of documents sent to the Pension Fund.

But an insured who in 2017 had less than 25 insured persons (including persons who entered into GPC contracts for which insurance premiums are charged) can take SZV-STAZH and EDV-1 on paper (clause 2 of article 8 of the Federal Law of April 1, 1996 No. 27-FZ).

Since EDV-1 is an annex to SZV-STAZH, it is, naturally, submitted within the same time frame. For 2017 - no later than 03/01/2018 (clause 2 of article 11 of the Federal Law of 04/01/1996 No. 27-FZ).

The procedure for filling out the EDV-1 form is given in Appendix 5 to the Resolution of the Board of the Pension Fund of January 11, 2017 No. 3p.

The EFA-1 form initially submitted must indicate the information type “Original.” To do this, place an “X” in the appropriate box.

When submitting EDV-1 with the SZV-STAZH form, you must fill out the following sections:

  • Section 1 “Details of the policyholder submitting the documents”;
  • Section 2 “Reporting period (code)”;
  • Section 3 “List of incoming documents”.

When submitting the SZV-STAZH form, which contains information about employees entitled to early retirement, in the EDV-1 inventory, you must also fill out section 5.

Section 1 is filled in by analogy with section 1 “Information about the policyholder” of the SZV-STAZH form (registration number in the Pension Fund of the Russian Federation, TIN/KPP of the policyholder and its short name are indicated).

Section 2 indicates the reporting period and its code in accordance with the Classifier (Appendix to the Filling Out Procedure, approved by Resolution of the Pension Fund Board of January 11, 2017 No. 3p). In EFA-1 for 2017 you need to indicate the code “0” and the year “2017”.

In section 3, fill in the column “Number of insured persons” - indicate the number of insured persons, information about which is indicated in the SZV-STAZH form.

Form SZV-STAZH- is a report on work experience, which must be submitted annually to the Pension Fund (for 2018, the report must be provided until March 1, 2019 of the year).

What is the SZV-STAZH form

Form SZV-STAZH is filled out and submitted by policyholders to the Pension Fund for all insured persons who are with the policyholder:

  • in labor relations(including with whom employment contracts have been concluded)
  • or who entered into civil law contracts with him, the subject of which is the performance of work, provision of services, copyright contracts, in favor of authors of works under agreements on the alienation of the exclusive right to works of science, literature, art, publishing license agreements, license agreements on granting the right to use works of science, literature, art, including remunerations accrued by organizations for managing rights on a collective basis in the benefit of the authors of works under contracts concluded with users, or with whom employment contracts and (or) civil law contracts have been concluded.

Changes in SZV-STAZH since 2019

Changes compared to the reporting for 2017 in the forms introduced by the Resolution of the Board of the Pension Fund of the Russian Federation dated December 6, 2018 No. 507P are minimal.

In the SZV-STAZH form, in section 3 in column 14, “information about the periods counted in the insurance period of the unemployed” was added to the previous title and the duplication of information on the policyholder disappeared.

In column 1 of section 3 of the form numbering is continuous, in ascending order, without gaps or repetitions.

The number is assigned to the record for a specific insured person. If the data on the period of work of the insured person contains several lines, the number is assigned to the first record for the insured person (in this case, “Last name”, “First name”, “Patronymic name”, “SNILS” are filled in once).

Surname and/or Name are required to be filled out. Surname may be absent, but required to indicate if present.

If it is necessary to reflect several periods of work for one person, each period is indicated on a separate line.

Work period- the dates must be within the reporting period specified in section 2 of the form.

For forms with the “Pension assignment” type, the “Period of work” column is filled in until the date of expected retirement.

Period of work of the insured person within the framework of a civil contract filled in with 11 codes reflected in column:

The SZV-STAZH form with the “pension assignment” type cannot be submitted for a period for which data has already been recorded on an individual personal account based on the SZV-STAZH form with the “Initial” or “Supplementary” type.

How to submit the form to the Pension Fund of Russia

SZV-STAZH can be sent via TKS or submitted on paper - in person, by mail or with a representative.

Policyholders who submit information to 25 people or more, are required to report only according to TKS. In this case, not only employees are considered, but also persons who have entered into civil contracts with the insured, payments under which are subject to insurance premiums.

Submission rules according to TKS:

  1. The document is signed by a strengthened CEP.
  2. The date of submission of information is considered to be the day when it was sent via TKS to the Pension Fund. This must be confirmed by a document from the EDF operator or the territorial body of the Pension Fund.

If the form is submitted on paper:

  1. Individual entrepreneurs put only a signature, organizations - a signature and seal (if available).
  2. Information on magnetic media can be attached to the paper form.
  3. If the form is sent by mail, then the day of submission is the date indicated on the postmark.

Sanctions for failure to submit

If the policyholder (organization or individual entrepreneur) does not submit the SZV-STAZH form on time, or it contains false information, he will be fined in the amount 500 rubles for each insured person.

If the form should be sent via TKS, but this order is violated (27-FZ), the fine will be 1000 rubles.

An administrative fine in the amount of from 300 to 500 rubles for failure to submit within the prescribed period or submission of incomplete and (or) false information.

Along with the SZV-STAZH form, you must submit the form. When submitting reports electronically, forms SZV-STAZH and EDV-1 are generated in one file.

Correcting SZV-STAZH errors

Today, organizations and individual entrepreneurs submit personalized information to insured persons in the RSV-1 calculation. And no separate forms with individual information are submitted to the Pension Fund. But if your company is already 20 years old or more, then, in principle, the Pension Fund of Russia at any time can ask you to provide additional information on those persons who have worked for you since 1996 and are now applying for a pension.

Why SZV-1

Over the past 20 years, the Pension Fund of Russia has approved many forms of personalized accounting, beginning with the abbreviation SZV, and a validity period was established for each. However, as a general rule, any personalized information submitted to the Pension Fund for previous years must be submitted according to the forms that were used in those years.

For this reason, if the Pension Fund asks you to submit information for 1996-2000 for one of the employees or former employees, you must use the SZV-1 form.

Where to get the SZV-1 form

The form itself can be downloaded, for example, from the Consultant+ system. In addition, there are free programs developed by the Pension Fund for preparing personalized accounting forms. Let's say Documents PU 5. It is also possible to issue SZV-1.

When submitting the form on paper, according to the rules, you must attach an inventory according to form ADV-6-1 to it. However, over the years, it is possible that PFR specialists will accept SZV-1 without it. In this case, it is not necessary to attach any other explanatory notes to the documents.
